If you are looking for facial feminisation surgery in Europe, you have probably already found the problem: the figures don't match. The same country appears at €12,000 in one guide and €30,000 in another. Some clinics publish prices, most don't, and the quotes you receive are built so differently that they can't be compared.

What moves the price from one country to another

‍

The difference is rarely in the surgery itself. It is usually in everything around it.

‍

The cost of running a clinic. Volume of salaries, premises and hospital fees vary a lot across Europe, and they end up in the price. So does structure: in-house facilities to pay off, international sales teams, travel coordination and marketing are part of what a patient pays, even though none of it happens in the operating theatre.

What the price includes. Some quotes are the surgeon's fee alone; others include hospital, anaesthesia, imaging and a year of follow-up. Two quotes several thousand euros apart can be the same price once you add up what each leaves out.

The technique. "Forehead surgery" does not mean the same thing in every quote. Burring down the bone (usually called Type I) does not have the same complexity, surgical time or fixation materials as opening and repositioning the anterior wall of the frontal sinus (Type III). If two quotes use the same name for two different surgeries, comparing them makes no sense.

The package. Hotel, transfers, a companion service. All of it has a cost, and it is not always itemised.

Turkey and the rest of Europe

Turkey is the alternative most patients weigh up nowadays, and its prices are generally lower. The question is not whether it is cheaper, but what the price covers and what happens after you fly home.

Many Turkish packages bundle surgery with hotel and transfers, and follow-up after the return is done remotely. That is not a problem in itself; it simply changes what you are comparing. Apply the same questions you would ask any clinic, in Turkey or anywhere else: who operates, which technique is used, what is included, and how revisions are handled if you need one.

Travel is part of the price

‍

For most European patients, FFS means travelling. Flights, accommodation and time off work belong in the comparison, and so does how long you need to stay.

At ARQÉA, the hospital stay is included in the price: for procedures that require admission, the first 24 to 48 hours after surgery. How long you stay in Barcelona after that depends on the procedure, and mostly on the stitches:

  • Tracheal shave: most patients travel home after about two days. We send you a video showing how to remove the suture yourself; it is a single thread that pulls out easily.
  • Jaw and chin: the surgery is done from inside the mouth and the stitches dissolve on their own, so there is nothing to remove.
  • Rhinoplasty: stitches come out after 7 days.
  • Forehead: stitches come out at 8 days. After the first night in hospital, most patients stay nearby for a couple more nights so we can check everything is going well. From the third or fourth day you can either stay in Barcelona or travel home and come back for the stitch removal.

Prices are VAT-exempt because they are healthcare services with a therapeutic purpose provided by physicians, exempt under Article 20.One.3 of the Spanish VAT Act: the published figure is the final price. The hospital nights shown are the usual ones and are adjusted to each case. The final quote is always given in writing after the assessment and the imaging study.

  • Included: surgical team fees, anaesthesia, operating theatre and post-anaesthesia recovery unit, surgical materials, CT scan and virtual surgical planning, hospital stay as shown in the table, and four follow-up reviews, in person or by video call: at 7 days, 1 month, 3 months and 12 months.‍

Who performs your surgery at ARQÉA

What happens if you need a revision

‍

Complications. Medical complications, when they occur, do so during the first year. If one occurs and further surgery is needed, we do not charge medical fees; the patient covers only the hospital, operating theatre and materials costs. An optional complications insurance covers all of those costs during the first year, whether or not there is a medical complication.

Revisions for the result. When there is no complication but the patient is not satisfied with the result, we assess the case with her. If a correction is justified, we do not charge fees for it either; hospital, operating theatre and materials costs are paid by the patient, unless she has taken out the insurance.

In both cases the reference period is the first year after surgery, applied flexibly when the case requires it.

What the price shouldn't decide

‍

Publishing prices carries a risk we want to name: that the figure becomes the criterion. It shouldn't. Facial feminisation is facial bone surgery. What should decide is the surgeon's experience in this type of surgery, the quality of the planning, and a plan that responds to your face rather than to a standard package. A low price without that is not cheap. And a high price, on its own, doesn't guarantee it either.

Feminisation is not the same as beautification: it works on the bone structures that make a face read as male, not on a general idea of beauty.
