In short: if you live in the UK and don't want to wait for the NHS, the realistic alternative is private facial feminisation surgery (FFS), in the UK or abroad. In Barcelona, Spain, a three-procedure FFS package at ARQÉA costs €16,000, with anaesthesia, hospital stay and four check-ups included, and you deal directly with the surgeon who operates, Dr. Javier Gutiérrez. Spain applies European standards: registered surgeons, licensed hospitals, written consent and the right to your medical records.
Most UK patients who write to us have done the maths already. The NHS route is measured in years, and a full private quote in the UK can run past €30,000. The question they bring is a different one: can I have this surgery in another country, safely, and still be looked after when I get home?
This guide answers that question with the figures and the rules, not with promises.
Why UK patients look abroad
NHS gender care starts with a referral to an adult gender dysphoria clinic. In December 2025, NHS England published an independent review of these clinics. By the end of March 2025, more than 40,000 people were waiting for a first assessment in England. In October 2024, the average wait had reached 296 weeks, about five years and seven months, and most clinics projected waits of 15 years or more if nothing changed.1
In Scotland, 6,435 people were waiting for a first appointment at a gender identity clinic on 31 March 2026, and 1,146 of them had been waiting for more than five years.2
And the clinic is only the first step. In England, facial feminisation surgery is classed as a non-core procedure and is not routinely funded: the only route is an Individual Funding Request on exceptional clinical grounds. We explain the policy in FFS on the NHS.
Your options if you don't want to wait
| Route | How long until surgery | Cost to you | Follow-up |
|---|---|---|---|
| NHS | Gender clinic first, with average waits of more than five years. Facial surgery only through an Individual Funding Request | None, if a request is approved | NHS |
| Private in the UK | Set by the surgeon | Published ranges of about €20,000 to €40,000 for full FFS | With the surgeon, in the UK |
| Private in Barcelona (ARQÉA) | Usually one to two weeks after you decide, once your tests are ready | From €4,500 per procedure. Three-procedure package €16,000, plus travel | Four check-ups in the first year, in person or by video call from home |
Is it safe to have FFS in Spain?
The NHS has its own advice for anyone considering surgery abroad: find out how surgeons and clinics are regulated, check the surgeon's training and experience, meet the surgeon before the operation, check the insurance and ask what happens if there is a complication.3 It is a good checklist. This is how each point works at ARQÉA.
| What the NHS advises you to check | At ARQÉA |
|---|---|
| How surgeons and clinics are regulated | Dr. Javier Gutiérrez is registered with the Barcelona College of Physicians (COMB), no. 08-43588. Surgery takes place at IM Clinic Gournay, Barcelona, a private hospital with the latest surgical technology. |
| Training, experience and number of procedures | Oral and maxillofacial surgeon, MD, PhD and Fellow of the European Board of Oral and Maxillofacial Surgery. Former senior surgeon at Facialteam for 11 years and founder of Facialteam Barcelona (2021 to May 2026). Over 17 years of experience and more than 1,050 facial procedures, trained at Vall d'Hebron University Hospital. |
| Who actually operates | Dr. Gutiérrez operates every facial feminisation procedure himself. Rhinoplasty is performed by Dr. Ramón Tarragona, a plastic surgeon, in the same session when it is combined with other procedures. |
| Meeting the surgeon before surgery | Your first consultation is with Dr. Gutiérrez himself, by video call and free. You see him again in Barcelona before the operation. |
| Not booking through sales staff | There are no sales coordinators between you and the surgeon. You receive a written, itemised quote, and it is final and binding. |
| Before-and-after results | We show real cases at the consultation, with each patient's permission, in date order and without retouching. |
| Language | The whole team works in English: consultation, documents, coordination and follow-up. |
| Insurance | Spanish law requires doctors in private practice to hold professional liability insurance.5 We also offer optional complications insurance for the first year. |
| Follow-up and complications | Four check-ups are included, at 7 days, 1 month, 3 months and 12 months, in person or by video call. The complication policy is explained below. |
Spanish law also protects you as a patient, wherever you come from. Surgery requires written informed consent, and you have the right to a copy of your medical records.4 That matters when you are back home: your GP can see exactly what was done.
If something goes wrong, Spanish law applies. You can make a complaint to the Barcelona College of Physicians, and the surgeon's compulsory professional liability insurance covers claims for damages.5
The checklist is the same one we recommend for any surgeon, in Spain or anywhere else. We go through it in more detail in our guide to choosing an FFS surgeon.
What you pay, from start to finish
Every price includes general anaesthesia and the surgical centre:
- Three-procedure FFS package, for example forehead, jaw and chin, and tracheal shave: €16,000
- Forehead feminisation: €8,500
- Jaw and chin: €7,000
- Rhinoplasty, performed by Dr. Ramón Tarragona: €7,000
- Tracheal shave: €4,500
Our prices are around 50% lower than those of other specialised centres. Published ranges for full FFS in the UK run from about €20,000 to €40,000. To compare like with like, add return flights and accommodation to our figure. Prices are in euros, and your bank converts them at the rate of the day you pay. We break down every cost in our guide to FFS costs in the UK and Europe, and our price calculator gives you an indicative range for your own case.
If the budget doesn't cover everything at once, start with the procedure that changes the most. We explain how to decide in which FFS procedure to do first.
How it works from the UK, step by step
- Free video consultation with Dr. Gutiérrez. You send photos beforehand, and you can ask everything you need.
- Written plan and final quote. What will be done, in which order, and the price that will be charged.
- Date and deposit. A 20% deposit reserves your date and fixes the price for six months. Dates are usually available one to two weeks after you decide.
- Tests near home. Pre-operative blood tests can be done at a laboratory in the UK, so the trip is only for the consultation days, the surgery and the recovery.
- Your stay in Barcelona. Flights from most UK airports take about two hours. How long you stay depends on the procedure: a tracheal shave is a day case followed by one night near the clinic, and forehead surgery needs about ten days. We explain it in our guide to travelling to Barcelona for FFS.
- Follow-up from home. Check-ups at 7 days, 1 month, 3 months and 12 months, by video call if you prefer. We explain how in our post on postoperative follow-up.
What if something goes wrong once I'm back in the UK?
Contact your coordinator straight away, with photos if you can. Dr. Gutiérrez reviews your case and decides the next step: a video call or a visit to a doctor near you, with a report for that doctor. If you need to come back to Barcelona, the team organises the trip with you. In an emergency, go to A&E or call 999 as you would for anything else, and let us know.
Medical complications, when they occur, do so in the first year. If one needs further surgery, we don't charge medical fees; you cover only the hospital, operating theatre and materials, and the optional complications insurance covers those too. If there is no complication but you are not satisfied with the result, we assess it with you, and if a correction is justified we don't charge medical fees for it either.
Frequently asked questions
Can I have FFS in Spain without waiting for the NHS?
Yes. Private surgery in Spain doesn't depend on the NHS gender pathway or on an NHS referral. You book a free video consultation with Dr. Javier Gutiérrez, and he assesses you directly. In some cases he may ask for a report from your doctor before surgery. Surgery dates are usually available one to two weeks after you decide, once your pre-operative tests are ready.
Is it safe to have facial feminisation surgery in Spain?
Spain applies European standards. At ARQÉA, the surgeon is registered with the Barcelona College of Physicians, surgery takes place at IM Clinic Gournay, a licensed hospital in Barcelona, and Spanish law requires written informed consent and gives you the right to a copy of your medical records.
Is Spain a good place to have FFS?
For UK patients it is a practical choice: European standards, flights of about two hours, a team that works in English and follow-up by video call from home. What matters most is still the surgeon and the hospital, so check them the same way wherever you go.
Which country is best for FFS?
There is no best country, only the right surgeon and hospital for your face. Check the surgeon's specialty, registration and number of FFS cases, where the surgery takes place, what the price includes and how follow-up works once you are home.
How long do I need to stay in Barcelona?
It depends on the procedure. A tracheal shave needs one night near the clinic, and you can fly home the next afternoon. Forehead surgery needs about ten days.
How much time off work will I need?
It depends on the procedure. Dr. Gutiérrez recommends about 2 days off after a tracheal shave or a lip lift, 2 weeks after jaw and chin surgery, and 3 weeks after forehead surgery or rhinoplasty. When procedures are combined, plan for around 3 weeks. Walking, talking and eating are normal from the first day; intense or structured exercise usually waits at least 4 weeks. If your work is very physical or depends on your appearance, it can take a little longer.
Can I have my follow-up from the UK?
Yes. The four check-ups, at 7 days, 1 month, 3 months and 12 months, can be in person or by video call, and pre-operative blood tests can be done at a laboratory near home.
Do I pay in pounds or euros?
In euros. Your bank converts the amount at the rate of the day you pay.
