Facial feminization surgery (FFS) is a group of operations that soften the features of a face that are read as masculine: the forehead and brows, the hairline, the jaw, the chin, the Adam's apple, the upper lip and the nose. At ARQÉA one surgeon, Dr. Javier Gutiérrez, assesses you, plans the bone work on your own CT scan and operates. The nose is operated on by Dr. Ramón Tarragona, surgeon at IM Clinic Gournay, within the same plan. Every price is published. The full pack, with forehead, jaw and chin, and tracheal shave, costs €16,000.
The FFS procedures, one by one
Each procedure has its own page, with the technique, the recovery and the price. Dr. Gutiérrez performs all of them except the nose.
- Forehead feminization, €8,500Softens the brow bone, the nasal root and the outer rim of the eye socket, and can raise the brows. Time off work: 3 weeks.
- Hairline lowering, €6,500Shortens the forehead by bringing the hairline forward, by up to around 3 cm. Time off work: 1 week to 10 days on its own.
- Jaw reduction, €5,000Lowers and narrows the lower jaw, softens the angles and slims the jawline, from inside the mouth. Time off work: 2 weeks.
- Chin reduction, €6,000Makes the chin shorter, narrower and softer by reducing and sculpting your own bone. Time off work: 2 weeks.
- Tracheal shave, €4,500Sculpts the thyroid cartilage so that it projects less, without touching the point where the vocal cords attach. Time off work: 2 days.
- Lip lift, €2,500Shortens the distance between the base of the nose and the upper lip. Time off work: 2 days.
- Nose feminization, €7,000Reshapes the bridge, tip and base of the nose. Performed by Dr. Ramón Tarragona, surgeon at IM Clinic Gournay. Time off work: 2 weeks.
Jaw and chin together cost €7,000. A lip lift costs €2,000 in the same session as other procedures. Hairline lowering with forehead feminization costs €8,500 with bone work, or €7,000 without it.

The full FFS pack: forehead feminization, jaw and chin, and tracheal shave, in the same operation. One price, stated upfront. The quote you receive after the consultation is itemized, final and binding, and valid for 3 months.

A 20% deposit reserves your date and fixes the price for six months, which can be extended to twelve. Complication insurance costs €250 and covers one reoperation, only in case of a complication. Where to start
Not everyone does every procedure, or does them all at once. These are the criteria we use with you at the consultation.
- The forehead firstThe gender reading of a face starts at the eyes, and the forehead is the roof of that region. On a limited budget, the forehead usually comes first.
- HormonesHormone therapy does not change the bone of the forehead or the cartilage of the Adam's apple, so you do not need to wait for it.
- CombiningForehead with jaw and chin, and the full pack, cost less than the same procedures separately. Combined procedures need about 3 weeks off work.
- Hair transplantA FUS transplant can be done in the same operation as the forehead. In another centre, wait at least 4 months after forehead surgery.
- The limitAnatomy sets the limit, not desire. When your anatomy allows less change, we tell you at the consultation, before operating.

Coming to Barcelona from abroad
Most of the preparation happens online, before you travel. How long you stay depends on the procedure.

FFS questions
How much does FFS cost in the UK compared with ARQÉA?
Private FFS prices in the UK vary from one surgeon to another, and they are higher than at ARQÉA. At ARQÉA single procedures start at €2,500 and the full pack costs €16,000, with the surgeon, general anaesthesia and the surgical centre included. Can I get FFS on the NHS or through my Krankenkasse?
At ARQÉA, FFS is a private procedure. We explain how it works in the UK in FFS on the NHS, and in Germany in FFS and the Krankenkasse. If your insurer asks for it, you receive an itemized invoice by procedure, in English.
Can everything be done in one operation?
Several procedures can be combined in the same operation, and the packs are priced that way. The nose can go in the same operation as forehead surgery and a tracheal shave, with Dr. Tarragona, surgeon at IM Clinic Gournay, operating in the same session. How many fit in one operation depends on you and on how complex your case is. As a rule, the full pack plus a hair transplant is about the most that goes into one operation.
How long do I need to stay in Barcelona?
It depends on the procedure. After a tracheal shave you can fly the next afternoon, and after jaw or chin surgery you stay two nights. After rhinoplasty the stitches come out at 7 days, and after forehead surgery on day 8, although you can go home on day 3 or 4 and come back.
How much time off work will I need?
About 2 days after a tracheal shave or a lip lift, 2 weeks after jaw, chin or nose surgery, and 3 weeks after forehead surgery. Combined procedures need about 3 weeks. You can go about normal daily life from day one, and intense exercise waits at least 4 weeks.
Is the consultation free, and can it be online?
Yes. The first consultation with Dr. Gutiérrez is free, in person at IM Clinic Gournay or by video call. You leave it with a written plan and a quote that is final, binding and valid for 3 months. If you go ahead, he always sees you in person in Barcelona before operating.
Does the price include anaesthesia and hospital?
Yes. Every price includes the surgeon's fees, the anaesthesia, the surgical centre at IM Clinic Gournay and the hospital stay, 24 to 48 hours depending on the procedure. Four check-ups are also included, at 7 days, 1 month, 3 months and 12 months.
